Required Steps to open a service request for equipment:

 

1. Download and complete the English or French CHS Service Request Form

2. Be sure to include your Repair Purchase Order on the form

3. Email your scanned completed form to service@chsltd.com

4. Wait for shipping instructions and a reference number from our service department email

5. Follow the shipping instructions provided, indicate the reference number on the CHS Service Request Form and on the outside of the shipping carton

6. Place a copy of the completed CHS Service Request Form in the shipping box with your equipment

7. Ship your item to the address as instructed. Use your facility’s regular shipping method.
