-
Why does the V Keeper remain exposed? Won't it avulse the vein branches?
While it is true that the V Keeper does not retract, its stationary design is actually an intentional safety feature used to protect the vessel, rather than a hazard.
Here is how it works to prevent avulsions during the procedure:
- Protective Guidance: As the dissector is advanced up the tunnel following dissection, the device is rotated by the operator.
- Shielding the Vessel: This rotation allows the V Keeper to gently guide the saphenous vein and its branches toward the bottom surface of the dissector.
- Minimizing Snags: By directing the vessel downward, the V Keeper actively keeps the tissue safely away from the V Cutter, which is the component far more likely to catch or snag on delicate branches.
Ultimately, the protruding V Keeper acts as a protective shield, safely routing the vein and branches away from the cutting mechanism.
-
The slot looks too small to fit a vein branch. How does it work?
The narrow design of the slot is actually intentional and essential for effectively sealing the vessel. Rather than needing to fit the full, round branch, the device is designed to compress it.
Here is exactly what happens as the branch enters the cutter:
- Blood Displacement: As the branch is guided into the slot, the device gently pushes the blood out of the way.
- Compression (Coaptation): Without the blood inside, the walls of the branch flatten and press tightly together.
- Clean Separation: The V Cutter then coagulates this flat, double layer of tissue and cleanly separates it.
As you continue to advance the V Cutter, it continuously repeats this smooth process—flattening, coagulating, and separating the branches safely and efficiently.
-
Why does your device torque the vein?
The VKeeper/V Cutter is designed to provide retraction of the saphenous vein and results in long branch lengths.
-
Why is grounding necessary?
Grounding the device is a critical safety step designed to protect the saphenous vein from heat damage during the procedure.
Here is how grounding works to ensure a safe and precise cut:
- Completing the Circuit: Pressing the large electrodes (located on the back of the V Cutter) against the tunnel wall safely completes the electrical circuit.
- Redirecting Energy: This setup forces the energy emitted by the active cutting electrode (on the front) to flow away from the delicate vein and into the surrounding tunnel wall.
- Protecting the Vein: By drawing the energy outward, grounding minimizes the “thermal spread” (the surrounding area affected by heat). This significantly reduces the risk of accidentally damaging the saphenous vein.
In short, grounding controls the flow of energy to ensure it only affects the intended tissue, keeping the main vessel safe.
